Health Department Offering Vaccine Waiting List

The Dade County Health Department will start taking a waiting list of names of those interested in taking the COVID-19 vaccine as it becomes available. The Dade County Health Department has partnered with Polk County Health Department to help bring vaccine to our counties.

Visit https://form.jotform.com/Polk_County/VaccineSurve to complete an online form. Please be sure to mark Dade as the county so it will come to Dade and not Polk. You may also call the Dade County Health Department to get on the waiting list.

Patients will be called as soon as vaccine becomes available. Thanks for your patience during this difficult time and as always, call the health department with any questions or concerns.
